Please have application filled out completely on day of clinic. No CASH will be accepted. Check/Money Order only made payable to THE TOWNSHIP OF HILLSIDE.
Please be reminded that if your pet's vaccination expires before October 31, 2021, you must get a new rabies shot before you file for the 2021 license. THIS IS A STATE LAW.
